Overview

Our Community Nursing team includes both General and Mental Health Registered Nurses, working alongside dedicated Psychosocial Recovery Coaches to provide comprehensive, person centred care.

We take a holistic approach that supports your physical and mental wellbeing. Whether you're navigating a new diagnoses or managing ongoing needs, we make clinical information easy to understand and help you establish, safe, sustainable health routines that work for your lifestyle.

Who it’s for

This service is designed for NDIS participants who require in home nursing care and mental health or psychosocial recovery coaching, in collaboration with their existing allied health and clinical teams.

What we do
  • Deliver nursing care for chronic conditions, wound management, diabetes, continence, and medication support.

  • Create easy to understand care plans, prompts, and checklists to keep you informed and in control.

  • Support you in building daily routines, staying motivated, and reconnecting with your community.

  • Help you work towards mental health goals such as managing stress, building confidence, and creating structure.

  • Provide respectful, trauma‑informed care tailored to your mental health and wellbeing goals; while involving family or carers in coaching if you wish.

How it works
  1. Getting to know you – We take the time to understand your story, health needs, and personal goals.

  2. Your care plan –Together, we co-design a clear, practical plan that firs your lifestyle and priorities.

  3. Visits and support – We provide regular in home visits and coaching to help you stay on track and feel supported.

  4. Check‑ins and next steps – We monitor your progress and collaborate with your wider care team to adjust your support as needed.

Funding & Access

Our Community Nursing Care and psychosocial supports are available under NDIS Support Item 0114.

You can access these services through your Core or Capacity Building Budgets, whether your plan is agency managed, plan manged or self managed.
